- 1
Name five kinds of written communication used in a health or social care setting.
Model answer
Care plans, daily records, handover notes, accident and incident forms, and referral letters are five kinds of written communication.
- 2
Give two advantages of written communication over spoken communication.
Model answer
A written record survives after the conversation ends and can be read by staff on a later shift. A record also provides evidence of what was decided and when.
- 3
Give two disadvantages of written communication.
Model answer
Written communication gives no immediate chance to check that the reader has understood. Anybody who cannot read the language or the format is excluded from it.
- 4
State four rules for writing in a care record.
Model answer
An entry must be legible, factual rather than opinion presented as fact, dated and timed, and signed by whoever wrote it.
- 5
Why should abbreviations be avoided in a care record?
Model answer
An abbreviation that is obvious to one team can be read differently by another, and a misread abbreviation about a dose or a diagnosis can cause direct harm.
- 6
What does a care plan set out, and why is a written one better than a spoken agreement?
Model answer
A care plan sets out a person's assessed needs, the support agreed to meet them, who provides it and when it is reviewed. A written plan reaches every worker on every shift, while a spoken agreement reaches only those present.
